Gift Preference Questionnaire and Card
A gift preference questionnaire asks a few easy questions, like favorite colors, snacks, hobbies, stores and things to avoid, so the person buying has something to go on. Fill in this free one, then print the card, email it or share a link. Your answers live in the link itself, not in an account.

What should a gift preference questionnaire ask?
The best questions are quick to answer and give a buyer something concrete: favorite colors, snacks and drinks, hobbies, books or music, stores they like, allergies, and a clear list of things to skip.
This one has twelve questions. Sizes and the wish list link are optional, so people can share only what they're comfortable with.
How do you use it for Secret Santa, a teacher or family?
Pick who the card is for and the wording adjusts. The coworker version suits Secret Santa and office gift swaps, the teacher version works as a favorite things list, and the family version is for relatives who ask what you'd like.
- Secret Santa: share the link in your group chat or print the card for the exchange.
- Teachers: send home a favorite things list so families can match a gift to your taste.
- Family: share it before birthdays and holidays so relatives skip the guessing.
How does the share link work, and is it private?
Your answers are packed into the part of the link after the # sign. We don't store them, and that part isn't sent to our servers when someone opens the link. Anyone you give the link to can read the card, so share it only with people you'd show it to anyway.
When someone opens a shared card, the page asks search engines not to index it.
What if a link looks broken?
Links that are cut off, edited or made by an older version of the tool show a short message instead of a card. Ask the person to create a fresh link from this page.
Frequently asked
Is the gift preference questionnaire free?
Yes. It's free with no account, and you can print the card or create a share link as often as you want.
Does Prezynt store my answers?
No. Your answers stay in your browser and inside the share link. If you email the card to yourself, Prezynt uses it only to send that email.
Can I use it for a Secret Santa questionnaire?
Yes. Choose the coworker version, fill it in, and share the link or print the card for your exchange. Everyone can keep their sizes private by leaving that question blank.
What goes on a teacher's favorite things list?
Treats, drinks, colors, hobbies, favorite stores and any allergies are the most useful. Choose the teacher version and the wording is already set for you.
Can I turn the answers into a wishlist?
Yes. Use the button under the card to start a wishlist in Prezynt, where you can save gifts from any online store and share the list.
